Where your information goes
The pilot application and waitlist use forms hosted on the Kurb website. When you submit one, the response is forwarded through a Cloudflare Pages Function to the Kurb intake spreadsheet managed by Ben. Ben receives an email notification so he can review the response. The website itself does not keep a separate public-facing copy of the submission.
Why we use it
Ben reviews questionnaire responses to understand your habits, goals, device interest, and whether Kurb is a suitable fit. If you are approved, the information may be used to contact you, arrange device delivery, support setup, coordinate the pilot, and follow up for feedback.
- Review pilot fit and available device formats
- Contact applicants about the next step
- Provide setup and pilot support
- Understand what works and what needs improvement
Information from the pilot device
If you participate, app and device records may support your weekly review and help us evaluate the prototype. These can include opens, skipped access windows, emergency unlocks, days without using, and schedule-following streaks. An opening is an access event—not a measurement of puffs, doses, potency, or how much you consumed.
What not to send
Please provide only what is needed to discuss pilot fit. We do not need passwords, pairing codes, payment or financial details, or detailed medical records. Please do not include them in a questionnaire or email.
Your choices
You can contact Ben with questions or to request a correction or deletion of application or contact information. We will review the request and respond. We do not currently publish a fixed public retention period for pilot records.
If we later want to publish an identifiable testimonial or case study about your experience, that should be agreed separately with you.
